AGED CARE AND REHABILITATION SERVICES
The Aged Care and Rehabilitation Services are part of the Medical Division and have been on the Governor Phillip Campus for many years.
DAY HOSPITAL
The Day Hospital consists of a team of medical and health care professionals who provide assessment treatment and support services to older people and disabled adults so they may live with maximum independence in their own homes for as long as possible.
Service Summary:-
Target Group: Persons aged 14 or over who require specialist rehabilitation treatment after acute illness or injury, older persons who have complicated medical conditions which interfere with their ability to manage in the community.
Service Provided: Day Hospital, Day Centre, Therapies (Physiotherapy, Speech Pathology, Occupational Therapy, Social Work), Staff Specialist Clinics (Geriatric Medicine, Rehabilitation Medicine, Amputees)
Hours: Enquiries 8am until 4.30pm Monday to Friday
Referral: Doctors referral required
THE PATIENT'S WORKSHOP
The Patient's Workshop operates Monday to Friday 8.30am until 4pm.
Service Summary:-
Target Group: Frail aged, people with disabilities including those with high dependency needs who are living at home.
Service Provided: Activities to enable clients to learn new skills, either working alone or as part of a team. Activities such as painting, sanding, welding, wood-turning and woodwork. Clients are also involved in socialisation.
Hours: 8.30am until 4pm
Referral: By health care worker
AGED CARE ASSESSMENT TEAM
The Aged Care Assessment Team is multi-disciplinary and provides assessment in the home, hospital or aged care facility. It operates Monday to Friday 8.30am until 5pm
Service Summary:-
Target Group: Older people, Frail aged, adults with disabilities and their carers
Service Provided: Assessment of care needs
Hours: Monday to Friday 8.30am until 5pm
Referral: By anyone, Doctors referral preferred
CADOP Dementia Day Centre
The CADOP Dementia Day Centre offers day services to dementia clients on Monday, Wednesday and Friday, 10.30am until 3pm
Service Summary:-
Target Group: Older people with dementia who can no longer copy in ordinary day centres, and their carers
Service Provided: Day Centre, carer support group
Hours: Monday to Friday 10.30am until 3pm
Referral: By anyone, Doctors referral preferred
HIV AND SEXUAL HEALTH PROMOTION UNIT
The HIV and Sexual Health Promotion Unit, Lemongrove Team is located in Gascoigne Street Penrith, in the grounds of Governor Phillip Hospiotal.
Service Summary: The team provides HIV, sexual health and Hepatitis C health promotion services for the community, organisations and staff of the area health service.
Target groups: priority groups include health care workers, gay and other homosexually active men, people living with HIV/AIDS, Aboriginal people, young people, injecting drug users, sex workers and people from diverse cultural backgrounds
Services provided: education and training, information and resources, health promotion activities, events and campaigns, contact for men's health issues
Other Services: the Drug and Alcohol Health Promotion team is also based at Lemongrove Unit.
Hours:- 8.30 to 5.00 Monday to Friday
PHYSIOTHERAPY
The physiotherapists provide a comprehensive service of assessment and treatment for people with movement or mobility problems. There is also a hydrotherapy pool with supervised programs.
Service Summary:-
Target Group: People referred to the day hospital with movement or mobility problems such as stroke, amputation, leg fractures, Parkinson's disease or MS.
Service Provided: Physiotherapy
Hours: Enquiries 8am until 4.30pm, Monday to Friday
Referral: Doctors referral required
SPEECH PATHOLOGY
The Speech Pathology service assesses and treats any problems with communication, problems writing or swallowing.
Service Summary:-
Target Group: Any Patient referred to the day hospital who has needs assesses and treats for any problems with communication, problems writing or swallowing.
Service Provided: Speech Pathology
Hours: 8.30am until 4pm
Referral: Doctors referral required
OCCUPATIONAL THERAPY
The Occupational Therapist provides a service of assessment, therapy programs education and home assessment
Service Summary:-
Target Group: Those patients referred to the day hospital needing assessment, therapy programs education and home assessment.
Service Provided: Occupational therapy programs, education and home assessment (Outpatient Clinic assessment carried out at Nepean Hospital)
Hours: Tuesday, 9.00am until 3.30pm
Friday, 9.00am until 5.30pm
Referral: Doctors referral.
SOCIAL WORK
The Social Worker provides counselling services to provide support and assistance for people experiencing difficulties as a result of illness or disability.
Service Summary:-
Target Group: For patients referred to the day hospital counselling, practical assistance, information on support groups, counselling for carers, assistance with applications to Centrelink etc.
Service Provided: Counselling, practical assistance, information on support groups, counselling for carers, assistance with applications to Centrelink etc.
Hours: Monday, Tuesday and Thursday
