Aged Care Assessment Team
Aged Care Assessment Team is a "Multi-disciplinary team providing assessment, information, advice and assistance to older people and their carers. to improve their quality of life, assist them to remain at home, or move into an aged care facility. Must live in Auburn Local Government Area."
Aged Care Assessment Teams are the only health professionals able to approve older people for hostels and nursing homes. The team comprises Medical (including a Geriatrician) and nursing staff (including a continence consultant) social workers, occupational therapists, physiotherapists, speech pathologists, and clerical staff. ACAT also runs a Carer Support Group, which is open to carers of all ages within the Auburn area.
Hours: 8.00 - 4.30 Monday - Friday (Public Holidays excluded)
